How It Works
1. Obtain your Requisition/ Order with your insurance information, diagnosis code and medical provider's information.
2. Book your appointment
3. Text or email your Requisition/Order form 24-48 hours before your scheduled appointment to:

4. We will come to you or you can come to our office and we will collect your specimen for a convenience fee.
5. We will drop your specimen off to your IN-Network lab for processing.
6. NO INSURANCE NO PROBLEM, we offer an affordable lab program. Book your appointment and send your requisition/order form via text /email.
