    The Pink Needle Medical Lab Services

    1. Acceptance of Terms


    By scheduling, receiving, or paying for services provided by The Pink Needle Medical Lab Services, you acknowledge that you have read, understood, and agree to these Terms and Conditions. These terms apply to all clients, patients, and visitors.

    2. Services Provided


    The Pink Needle Medical Lab Services provides mobile and/or onsite phlebotomy, specimen collection, laboratory-related services, DOT and Non-DOT drug testing, and other authorized medical services as permitted by law.

    We do not diagnose, treat, or prescribe medical care.

    3. Medical Disclaimer


    All laboratory testing and specimen collection services are performed based on physician orders, employer requirements, or client requests where legally permitted. The Pink Needle Medical Lab Services is not responsible for medical decisions made based on test results.

    4. Patient Responsibility


    You agree to:

        •    Provide accurate personal, medical, and  any medication you talking that could affect the draw (such as Blood Thinners, Aspirin, etc)

        •    Follow pre-test and post-test instructions

        •    Inform staff of any medical conditions, allergies, fainting history, or complications related to blood draws 



    Failure to disclose relevant information may increase the risk of adverse outcomes.

    5. Consent to Treatment


    By receiving services, you voluntarily consent to specimen collection (including blood, urine, saliva, or other samples) by qualified personnel.

    You understand that minor risks may include bruising, bleeding, dizziness, infection, or discomfort.

    6. HIPAA Privacy & Confidentiality


    The Pink Needle Medical Lab Services complies with the Health Insurance Portability and Accountability Act (HIPAA).

    Your protected health information (PHI) will only be used or disclosed for treatment, payment, operations, or as required by law.


    You may request access to your records or request restrictions as permitted by HIPAA regulations.

    7. DOT & Non-DOT Drug Testing


    For DOT and Non-DOT drug testing:

        •    Clients must present valid identification

        •    Refusal to test, adulteration, or substitution will be documented

        •    Results will be reported to the designated employer, Medical Review Officer (MRO), or authorized entity


    DOT testing follows all applicable federal regulations.

    8. Scheduling, Cancellations & No-Shows

        

    •    Appointments must be scheduled in advance through our website or by phone. Same day appointments are subject to availability.

        •    Cancellations/ recshedules  of appointments must be made within 24 hours of scheduled appointment

        •    Failure to cancel or reschedule  may result in a no-show fee

    9. Payments & Fees


    Payment is due at the time of service unless prior arrangements have been made

    Accepted payment methods may include cash, credit/debit cards, or electronic payment platforms.

    Prices are disclosed at the time of service and may vary based on services rendered.

    10. Results & Turnaround Times


    Laboratory result turnaround times are estimates and may vary based on:

        •    Test type

        •    Laboratory processing

        •    Holidays or unforeseen delays


    The Pink Needle Medical Lab Services is not responsible for delays caused by third-party laboratories.

    11. Limitation of Liability


    To the fullest extent permitted by law, The Pink Needle Medical Lab Services shall not be liable for indirect, incidental, or consequential damages arising from services rendered, except in cases of gross negligence or willful misconduct.

    12. Refusal of Service


    We reserve the right to refuse or discontinue services for:

        •    Safety concerns

        •    Non-compliance

        •    Abusive or inappropriate behavior

        •    Failure to provide accurate information

    13. Governing Law


    These Terms and Conditions are governed by the laws of the State of Alabama, without regard to conflict of law principles.


    14. Changes to Terms


    The Pink Needle Medical Lab Services reserves the right to update or modify these Terms and Conditions at any time. Clients are encouraged to review these terms periodically for any modifications. By scheduling an appointment  with our phlebotomy staff, the client agree to adhere to these terms and conditions.
